Abstract
Summary. Systemic and selective thrombolysis is one of the commonly used methods for treating patients with acute vascular pathology, such as ischemic stroke, myocardial infarction, acute ischemia of the lower extremities. However, in some cases its use does not lead to the desired results and may be accompanied by life-threatening complications, including major and minor bleeding. To date, in clinical practice there is a lack of clear criteria for determining the clinical and technical effectiveness of thrombolysis, on the one hand, and the risk of hemorrhagic complications, on the other hand. The blood clotting system plays an important role in the development of these complications. Currently, there is an expanding understanding of the role of the primary markers of the blood coagulation system in the pathogenesis of many cardiovascular diseases. Understanding the functioning and interaction between coagulation and fibrinolytic blood systems in acute vascular pathology and thrombolysis can reveal predictors of adverse outcomes, which, in turn, will contribute to improving the effectiveness and safety of this treatment. This article presents a comprehensive overview of various blood clotting factors and markers studied in the context of thrombolysis outcomes in patients with acute vascular pathology. It is concluded that laboratory definition of von Willebrand factor, factors VIII, XIII, VII, thrombin production, and thrombin-antithrombin complex should be expanded for more accurate prediction of thrombolysis outcomes.For citation: Klimentova E.A., Kalinin R.E., Pshennikov A.S., Egorov A.A., Suchkov I.A.
